We do accept most insurance plans, however, with the constant changes to the insurance industry, this may be subject to change. To ensure that we are providers with your plan, please contact our office (305) 273-9377 for more information.
We will cooperate fully with patients’ insurance carrier; but, it is important that you are aware of any restrictions specific to your plan. Please call your insurance carrier with questions regarding your coverage, co-pays, co-ins, and deductibles.
Please remember if your insurance requires a referral it will be the patient’s responsibility to request this from his/her primary care physician and to bring it with them to the appointment. This will avoid delays, and we will not be able to see you without the referral mandated by your insurance company.

Patients are responsible for paying deductibles and co-payments at the time of their visit. To make payment convenient, we accept all major credit cards, cash or checks.
